Repairing Fractures Alternative Way Using Flexible Steel Wire

Introduction: As the costs of materials and methods of internal fixation are constantly increasing, the budgets available are not. A ideal way to treat hand fratures should also consider that so. An alternative way to treat fractures is using cerclage. Besides its traditional presentation, authors describe original variations. Methods: The authors show pictures of hand fractures radiographs before and after treatment, using circular cerclage and its original variations: simple cerclage and partial circular cerclage. Surgical method is also presented by pictures . Results: Applying cerclage original variations some kinds of fractures can be treated : tranverse, short oblique, long oblique and cominnuted ones. Simple cerclage refers to its use to repair transverse fracture by unitting dorsal cortical only (and allowing flexor tendon strenght to stabilize volar cotical). Partial circular cerclage refers to unitting short or long oblique fragments without being necessary to circulate them, by drilling mid lateral corticals and passing the flexible wire trought it. Cominnuted fracture is shown treated by circular cerclage. Conclusion: Despite not being an rigid internal fixation and do not permitting early motion its low cost and easy way of using grant to cerclage (circular or variations) an available method to treat fractures. It also permits soft tissue preservation due to its low implant profile. And it has to be considered as an additional method to be used alone or in association to other methods.
